Transitions between services in people with Intellectual Difficulties
  The project focuses on person centred care for people with learning disabilities at the point of transition from child to adult health services. The project will include multiple nursing perspectives and is based on an innovative initiative involving, learning disability liaison nurses, primary care practice nurses and general hospital nurses, and will be of national and international significance. It involves a project that will identify current nursing best-practice at the time of transition for young people with complex, long-term health needs that will inform the development of new nursing practice and will lead to the development of an education development programme to enable and promote long term change in practice, thereby improving patient care and outcomes.
