Development of a Patient Centred Assessment tool for hospital discharge planning among ICU survivors: PCAM-ICU
  People who survive Intensive Care (ICU) have complex support needs due to pre-existing illness and the new, long‐term impact of critical illness. Hospital discharge planning currently fails to identify these patients’ needs, impacting on community recovery (including high rates of hospital readmission) and access to health and social care support. We will therefore work with clinical experts, patients and families to develop and test an ICU‐specific version of a validated assessment process that identifies support needs among patients with complex illnesses. We’ll seek follow‐on funding to explore its UK‐wide implementation in hospital discharge planning and its
impact on patient recovery.
