Anonymous Medication Reporting in a District General Hospital,
  Anonymous reporting of medication errors was introduced into a district general hospital as a short-term pilot project. For the duration of the project this system ran in parallel with the existing official system, where staff reporting medication errors do not retain anonymity. The pilot project aimed to compare the number of errors reported anonymously with those reported via the official system during the same period. The results clearly show that the official error reporting system was ineffective when compared against one where anonymity can be retained.
